Erectile dysfunction ( ED ) may refer to difficulty in achieving an erection, difficulty in sustaining or maintaining an erection or both. In milder forms, it may also present as unsatisfactory hardness of the penis during erection.
Problems with erection can happen occasionally in otherwise healthy and young men. Excessive alcohol, fatigue and psychological factors such as stress or recent loss of a loved one are major causes. Infrequent occurrences may not require any treatment other than reassurance.
However, if this happens 50% of the time or more, there may be a problem which requires treatment. Risk factors include conditions like diabetes or peripheral neuropathies as well as trauma and injury to the spine or spinal nerves. Smokers and obese men are also more likely to suffer from erectile dysfunction.
The IIEF-5 ( International Index of Erectile Function ) questionaire is a useful screening tool to determine if you may have ED. Please give your responses based on your experience in the past 6 months.
A score between 17 to 21 may indicate mild ED.
A score between 12 to 16 may indicate mild to moderate ED.
A score between 8 to 11 may indicate moderate ED.
Patients who score 7 or less are likely to have severe ED.

Premature ejaculation occurs when a man reaches orgasm or climax early ( prematurely ) and consequently expels semen ( ejaculates ). The key questions to ask when trying to determine if you may have premature ejaculation are :
1) Do you ejaculate / orgasm / climax before you wish to? And…
2) Does this result in frustration or distress to you or your partner?

It is common to find factors like stress, anxiety, strained relationships, ’emotional baggage’ and other emotional or psychological conditions in patients with premature ejaculation. In patients where the exposure to such psychological stressors are prolonged, premature ejaculation may persist even when the initial source of stress is gone. Hence, premature ejaculation should be seen not only as a condition to be dealt with in its own right, but also potentially a symptom of ongoing psychological stressors which deserve to be examined.
ANDROPAUSE ( MALE HORMONE DEFICIENCY )
The term ‘Andropause’ has been used for some time now to refer to the man’s equivalent of a woman’s ‘menopause’. In both situations, the key event is the drop in hormone levels which then triggers a series of changes in the physical ( and even emotional and psychological ) self.
However, unlike menopause, where cessation of menstruation offers a relatively convenient way to gauge onset, andropause tends to happen more insidiously and often without symptoms which link directly to the male hormones. Some of the symptoms which patients may first complain of include :

Appearance of 1 or more of the above symptoms can also be due to other conditions apart from low testosterone or andropause. To differentiate the various causes, it may be useful to undergo some form of health screening or at least perform testosterone testing. Hair loss may occur in both men and women, but tends to be more common in men.
In men, the commonest treatable cause of hair loss is the so-called ‘male pattern hair loss’ or alopecia androgenetica. Medically proven treatments for alopecia androgenetica usually involves the use of topical agents like Minoxidil and / or oral medication like Finasteride ( Propecia ). These treatments can be started after the doctor excludes other ( less common ) conditions which may also cause hair loss, such as alopecia areata, fungal or bacterial infections of the scalp and thyroid problems. Autoimmune conditions such as SLE ( lupus ) are rare in men but may also cause hair loss.
As it is usually easier and more effective to slow or stop hair loss than to regain lost hair, it is advisable to seek medical advice early if you suspect yourself of having a problem with hair loss.
